In Lepanam, herb-based pastes (made from finely ground powders mixed with oil, milk or herbal decoctions) are applied directly to specific areas of the skin. The paste is left on for a set time (often 30–60 minutes), allowing the herbal ingredients to penetrate the skin and tissues. Common formulations include turmeric, sandalwood, neem and other medicinal herbs chosen for the condition being treated.

It cools and soothes inflamed or painful areas, and is especially beneficial for skin problems and joint inflammation. For example, a Lepanam paste might be applied to an arthritic knee, a gouty joint or a painful muscle strain to help reduce swelling and ease pain. It also helps heal skin ailments — Lepanam is used to treat eczema, psoriasis, acne and dark pigmentation by cleansing and nourishing the skin.
By delivering potent herbs directly to the troubled area, Lepanam relieves pain and inflammation while promoting tissue healing. It is often used alongside other therapies in Ayurvedic treatment plans for its targeted, detoxifying action.
